How to Download and Email the Intake Form
Option 1: Print, Fill Out, Scan, and Email
1. Download the PDF:
- Click on the link provided above to download the form.
- The form will either open in a new tab or download to your computer.
- If it opens in a new tab, click on the download icon (usually looks like a downward arrow) to save it to your computer.
2. Print the Form:
- Open the PDF from where it’s saved on your computer.
- Click on the print icon or press Ctrl + P (Windows) or Cmd + P (Mac) to print the form.
3. Fill Out the Form:
- Use a pen to complete all required sections on the printed form.
4. Scan the Completed Form:
- Use a scanner to scan the completed form back into your computer.
- Save the scanned document with a new file name (e.g., "FirstLastName_Intake_Form.pdf").
5. Email the Completed Form:
- Open your email program and compose a new email.
- Attach the scanned PDF file and send it to kinetickidsot@gmail.com.

- You can use Adobe Acrobat Reader (free), Microsoft Edge, or another PDF-filling software like PDFescape, or Preview (Mac users) to open the form.
3. Fill Out the Form:
- Use the software’s tools to click on each field and type in your information.
- Double-check all your entries for accuracy.
4. .Save the Filled-Out Form:
- Once you’ve completed the form, save it with a new file name (e.g., "FirstLastName_Intake_Form.pdf").
5. Email the Completed Form:
- Open your email program and compose a new email.
- Attach the filled-out PDF file and send it to kinetickidsot@gmail.com.
